Update to Form I-693, Report of Medical Examination and Vaccination Record. New edition dated 03/30/15.
Generally, all applicants filing for adjustment of status to that of a lawful permanent resident must submit Form I-693 completed by a designated civil surgeon. Form I-693 is used to report results of a medical examination to USCIS. The examination is required to establish that an applicant is not inadmissible to the United States on public health grounds. A list of those health grounds can be found in section 212(a)(1) of the Immigration and Nationality Act.
Update to Form I-698, Application to Adjust Status from Temporary to Permanent Resident (Under Section 245A of the INA). New edition dated 03/30/15.
This form is used to apply for adjustment to permanent resident status under the amnesty program provisions of the Immigration Reform and Control Act of 1986 (IRCA).
Update to Form I-687, Application for Status as a Temporary Resident Under Section 245A of the Immigration and Nationality Act. New edition dated 03/30/2015.
For use during the Immigration and Nationality Act (INA) Section 245A legalization program of the 1986 Immigration Reform and Control Act, which ended in 1988. The form is now used to apply to USCIS for benefits under the terms and conditions of certain settlement agreements.
Update to Form I-130, Petition for Alien Relative. New edition dated 03/23/15.
For citizen or lawful permanent resident of the United States to establish the relationship to certain alien relatives who wish to immigrate to the United States. Note: A separate form must be filed for each eligible relative. USCIS processes Form I-130, Petition for Alien Relative, as a visa number becomes available. Filing and approval of an I-130 is only the first step in helping a relative immigrate to the United States. Eligible family members must wait until there is a visa number available before they can apply for an immigrant visa or adjustment of status to a lawful permanent resident.
Update to Form I-924, Application For Regional Center Under the Immigrant Investor Pilot Program and Form I-924A, Supplement to Form I-924. New editions dated 03/18/15.
This form is used to: 1. Apply to the U.S. Citizenship and Immigration Services (USCIS) to request designation of an entity to be a Regional Center under the Immigrant Investor Pilot Program. 2. Request approval of an amendment to a previously designated Regional Center. An amended regional center designation request may include requests for determinations relating to any or all of the reasons for filing an amendment request noted below. A. An amendment request may be filed to seek approval of changes to the Regional Centers: 1. Geographic area; 2. Organizational structure or administration; 3. Affiliated commercial enterprise investment opportunities, to include changes in the economic analysis and underlying business plan used to estimate job creation for previously approved investment opportunities and industrial clusters; 4. Affiliated commercial enterprise's organizational structure and/or capital investment instruments or offering memoranda. B. An amendment may also be filed to seek a preliminary determination of EB-5 compliance for documentation provided as an exemplar Form I-526, Immigrant Petition by Alien Entrepreneur, prior to the filing of Form I-526 petitions by individual alien entrepreneurs.
Update to Form I-929, Petition for Qualifying Family Member of a U-1 Nonimmigrant. New edition dated 03/16/15.
To request immigration benefits on behalf of a family member who never held U nonimmigrant status.
Update to Form I-854, Inter-Agency Alien Witness and Informant Record. New edition dated 03/13/15.
Law enforcement agencies (LEAs) use Form I-854A to request an alien witness and/or informant receive classification as an S nonimmigrant. LEAs use Form I-854B to request an alien in S nonimmigrant status be permitted to apply for adjustment of status to adjust to lawful permanent resident (LPR) status under section 245(j) of the Immigration and Nationality Act (INA).
Update to Form I-539, Supplement A, For persons seeking V nonimmigrant status while in the United States or extension of V status. New edition dated 03/11/15.
This form is used by certain nonimmigrants to extend their stay or change to another nonimmigrant status; CNMI residents applying for an initial grant of status; F and M nonimmigrants to apply for reinstatement; and, Persons seeking V nonimmigrant status or an extension of stay as a V nonimmigrant.
Update to Form N-648, Medical Certification for Disability Exceptions. New edition dated 03/11/15.
This form is intended for applicants for U.S. citizenship who seek an exception to the English and civics testing requirements for naturalization "because of physical or developmental disability or mental impairment."
Update to Form G-28, Notice of Entry of Appearance as Attorney or Accredited Representative. New edition dated 03/04/15.
